Fetal Circulation

Definition 

Fetal circulation is the specialized circulatory system in a developing fetus that allows blood to bypass the non-functioning lungs and instead rely on the placenta for oxygen and nutrient exchange. It includes unique structures like the ductus venosus, foramen ovale, and ductus arteriosus that direct blood flow efficiently until birth.

Differences Between Fetal and Postnatal Circulation:


Changes After Birth (Closure of Fetal Shunts):
After birth, the baby starts breathing, and major changes occur in circulation:
1. Foramen Ovale
Function: Allows blood to flow from right atrium to left atrium.
After birth: Closes due to increased left atrial pressure → becomes fossa ovalis.
2. Ductus Arteriosus
Function: Connects pulmonary artery to aorta, bypassing lungs.
After birth: Closes within hours to days due to oxygen exposure → becomes ligamentum arteriosum.
3. Ductus Venosus
Function: Bypasses liver, directing blood from umbilical vein to IVC.
After birth: Closes → becomes ligamentum venosum.
4. Umbilical Vein
Function: Carries oxygenated blood from placenta.
After birth: Closes → becomes ligamentum teres (round ligament) of liver.
5. Umbilical Arteries
Function: Return deoxygenated blood to placenta.
After birth: Proximal part remains (internal iliac branches), distal part → medial umbilical ligaments.


Role of the Placenta in Fetal Circulation


The placenta acts as the lifeline between the mother and fetus. It performs several essential roles in fetal circulation:
1. Gas Exchange :-  Oxygen from the mother’s blood diffuses into the fetal blood.
Carbon dioxide from fetal blood diffuses into the mother’s blood for removal.
This replaces the role of lungs in the fetus.

2. Nutrient Supply :- Transfers essential nutrients (glucose, amino acids, fatty acids, vitamins) from maternal to fetal blood.

3. Waste Removal :- Removes fetal metabolic wastes (urea, creatinine) into maternal blood for excretion.
4. Immune Protection:-  Transfers maternal antibodies (IgG), giving the fetus passive immunity before birth.

5. Hormone Production :- Produces hormones like hCG, progesterone, and estrogen to maintain pregnancy and support fetal growth.

6. Blood Flow Pathway :- Oxygenated blood flows from the placenta → umbilical vein → fetal circulation.
Deoxygenated blood returns from the fetus → umbilical arteries → placenta.
